Once again this year we will be offering health clinics at DCA. Please support the clinics again this year, on Sunday, April 29th. We will have most of the forms online so you can fill out the paperwork ahead of time.

  Please fill out the top of the form. We will be sending all forms together to receive a reduced rate. Your $25.00 microchip fee includes the registration fee.
